Birth Registration

Parents can now register their children, under the age of one year using the Newborn Registration Service at www.serviceontario.ca/newborn. This electronically registers the birth directly with the Office of the Registrar General.  This will allow you to receive a Birth Certificate, Social Insurance Number and apply for your baby's Canada's Child Benefits. By using the Newborn Registration Services, it allows for quicker processing time and the convenience of online transactions.

Statement of Live Birth Forms can still be completed and mailed to the Office of the Registrar General.  Please complete the form in black or blue ink ONLY.  If an error is made on the form, simply stroke a line through the error and both parents must initial the change.  Do Not use correction fluid or the form will be Void. Statement of Live Birth Form and Birth Certificate Application can be mailed together.

Birth Certificate Applications

A Birth Certificate Application Form can be obtained at the Municipal Office or by downloading the form from the Service Ontario Website, Birth Certificate Form.  The Office of the Registrar General through Service Ontario allows you to complete the birth certificate application form online, On-Line Birth Certificate Application.

Death Registration & Death Certificates

To register a death, a family member and the Funeral Director complete the Statement of Death with information about the deceased.  The Funeral Director submits the Medical Certificate of Death and the Statement of Death to the municipality.
